The Fulton-DeKalb Hospital Authority has remained dedicated for over seven decades to ensuring that the communities we serve have access to quality care. We have made it our mission to decrease health disparities and promote the general health of the communities we serve through advocacy, education, and outreach.

Funding Request Form

If you are interested in applying for funding for your community program, please complete this form and forward to FHAfunding@thefdha.org . As we have several funding programs, submission of this form will determine whether you are eligible to apply for funding under any of the programs.  If your community program meets any of the eligibility requirements, the appropriate application will be forwarded to you for completion. If your application does not meet any of our program requirements, you will be advised accordingly. 

The FDHA will review your request and respond within 48 hours. Completion of this form or any subsequent application does not guarantee funding from The FDHA.

The FDHA reserves the right to deny funding that does not align with our mission, goals and areas of focus.
